Need more reasons to get vaccinated?
If you’re not yet vaccinated against COVID-19, there’s never been a better time than now. Highly contagious variants of the virus are spreading more easily. This is further increasing the risk of severe illness, hospitalization and death in unvaccinated people – especially individuals who are 65 and older. If you’re already vaccinated, we urge you to get the booster shot now. Data shows that the initial COVID vaccination loses effectiveness over time and the booster is critical to avoiding serious illness and hospitalization. Vaccination works.
How to get your $25 gift card
Simply send us a copy of your completed vaccination card (showing both doses of the Pfizer/Moderna vaccine or the single dose of Johnson & Johnson and, if applicable, the booster shot). There are three easy ways to provide your completed vaccination card. Pick the one that’s best for you:

Once the copy of your vaccination card is received, we will mail you your gift card to the address we have on file within 6-8 weeks.
Members have until 1/31/2024 to respond to the 2023 program.

* Covid-19 vaccination incentive can only be earned once per lifetime for the initial vaccine series. The booster shot incentive can be earned once per calendar year.
